OBJECTIVES:Upon completion of this program, participants should be able to: 1. Recognize roles and benefits of multidisciplinary heart failure (HF) disease management program in improving outcomes of patients with HF. 2. Understand the goal of HF care e.g. decreasing mortality, improving quality of life and preventing heart failure readmission. 3. Identify roles of key members of the HF team which comprises of doctors (primary care physician, Internist, or cardiologist), nurses, pharmacists, dieticians, and physical therapists. 4. Effectively educate patients with HF for self-care, diet, salt, fluid, signs and symptoms to call, medication, activities, and follow up. 5. Describe roles of, demonstrate, and interpret of common tools in HF clinic such as 6-minute walk test. 6. Effectively initiate individualized protocols for patients with HF at a local institute. 7. Recognize common pitfalls in setting up heart failure disease management program and describe how to overcome. 8. Recognize heart failure networking and role of care sharing between public, academic and private sectors. 9. Demonstrate decision–making when to refer patients with refractory/advanced HF for further management such as mechanical circulatory support and heart transplant. * Specific topics or areas of focus can be arranged upon request.
